    This cystic lesion in the right lower abdominal quadrant could be seen prenatally. Postnatal confirmation of the sonographic findings. Clinically, this female patient was unremarkable, except for a soft resistance was palpable.

    Ultrasound 1: Right lower abdominal quadrant with large cystic mass and sedimentation.





    Ultrasound 2 <- view Ultrasound 2

    Ultrasound 2: Right lower abdominal quadrant with large cystic mass and sedimentation and a central, non-echogenic structure.





    Ultrasound 3 <- view Ultrasound 3

    Ultrasound 3: Right lower-middle quadrant, cystic mass close to the urinary bladder, thin walled.

    Since the mass was large and regression was absent, the indication for laparotomy was present. Intra-operatively, a filled, elastic mass (about 5x7 cm) which had the color of creamed coffee, was found. This mass was filled with coffee-grounds-like content. The vessel stem, which was supplying nutrients to the mass, was torqued. The surrounding adnexes were also infarcted.
